The Child and Youth Mental Health Outreach Program provides services to children and youth under the age of 19 years with mild to moderate mental, emotional and behavioral difficulties, and their parents or caregivers so they can function to their highest capacity. The Child and Youth Mental Health Outreach Program serves the Elk Valley including Fernie, Sparwood, Elkford and Jaffrey.

What Our Program Does:

• Seek to increase individual and community awareness of child and youth mental health issues
• Try to increase healthy lifestyle choice for children, youth and their families
• Seek to increase the stability in lives of children and youth
• Provide a safe and supportive environment for children and youth to explore their feelings, behaviors, values and beliefs
• Create an environment in which children and youth can participate in and be actively involved in setting their goals for treatment

Desired Outcomes:​

1. Children and youth will have an increased capacity to function independently
2. Children and youth are actively involved in the decisions that are made which affect their lives
3. Increased resilience to promote positive mental health

FAMILY & CLIENT SERVICES

• One on one sessions with the child or youth
• Phone or in-person support to family members when needed
• Pro-active efforts to improve the quality of life for children, youth and their families
• Connection to appropriate community resources
• Access to timely and appropriate interventions for children, youth and their families

REFERRAL PROCESS

Self-Referral

Contact CYMH Outreach through email to receive the referral form. Complete and return this form. You will be contacted soon by the Outreach Worker to schedule an intake meeting.

Referral by Community Agency

Receive permission from referred family prior to submitting referral form. If needed, contact CYMH Outreach to receive referral form, complete and return the referral form. The family will be contacted soon by the Outreach Worker to schedule an intake meeting.
